Online TVA Personal History Questionnaire (PHQ) Instructions
1. Go to: https://conhire.tva.com
2. Click on Non-R30 Personal History Questionnaire.
3. Fill out all information on the PHQ.
4. Once the PHQ is completed, click the "Submit" button.
5. When the PHQ is submitted, a screen will come up with the following statement:
"Thank you, ___ for your online PHQ submission that was submitted on MM/DD/YYYY"
(You may want to print this page out and bring it with you as a confirmation of submission)
